If you pass away, we want to ensure your beneficiary or family can easily navigate the claims process during a difficult time. This email outlines what they need to know and do to file a claim with MetLife Worldwide Benefits.

How to get started

For Life claims, your employer or beneficiary should submit a claim to MetLife. The first part of the claim form needs to be filled out by the policyholder (employer) and the second, by the beneficiary. Typically the employer will start this process.

What happens next

After your beneficiary submits the claim, we reach out to confirm receipt and explain the support services that are available as part of the plan. Here’s what happens next:

  • Processing – We process the claim. If additional information is needed, we’ll contact the submitter. Our goal is to process claims within 10 business days once all documentation is received.
  • Confirmation – Once the claim is processed, we contact the beneficiary to confirm the claim was processed, explain how the claim was paid, and reiterate any additional support services they may have access to.
  • Payment – Payment is issued according to the beneficiary’s preferences. We can pay in nearly any currency to bank accounts around the world. We will follow-up with the beneficiary to confirm payment was received and ensure no lingering questions.

Other Life forms available

Other Life forms that are available for download on the Claim Form page include: 

  • Evidence of Insurability (EOI) form – If the coverage amount you request exceeds the Guaranteed Issue, Evidence of Insurability is required. Complete this form if you are requesting a Life insurance amount above your Guaranteed Issue (GI). Read more about Evidence of Insurability.
  • Beneficiary form – A life insurance beneficiary is the person or entity you designate to receive the death benefit from your policy. To designate a beneficiary, you may use either your company’s beneficiary form or our form below. Once completed, please submit the form to your employer. MetLife will confirm beneficiary information with your employer at the time of claim – you do not need to submit the form to use before then.
  • Conversion form – If you’re facing changes that will decrease or end your Life insurance with MetLife, you may have the option to ensure continued protection for you and your family. Complete this form if you would like apply to convert your policy to an individual policy so that you don’t lose your coverage. Read more about Conversion
